Rotherham United were the winners at Wembley with a 2-1 win over Shrewsbury Town in extra time, meaning they will join League One winners Wigan Athletic and runners up Blackburn Rovers in the Championship next season.
All three teams have made an immediate return to the Championship following relegation from the second tier at the end of the 2016/17 season.
The other three teams to complete the division are West Bromwich Albion, Stoke City and Swansea City who were relegated from the Premier League earlier this month.
