‘Game Plan’ is a new intervention project focused on empowering adults aged 18-25 who are facing challenges with their mental and emotional wellbeing to develop the skills, confidence, and knowledge to thrive and live their best lives.

‘Game Plan’ will offer accessible community-based provision that complements Crisis Teams and Adult Mental Health Services at a time when vulnerable adults are requiring support more than ever before. The programme has been designed to help meet increased demand for intervention services within communities, entailing activities that are bespoke and beneficiary led.

The role will operate an internal and external referral process whereby the Health and Wellbeing Officer will work with key staff and partners including Lancashire and South Cumbria NHS Foundation Trust to support pathways and successful on-boarding into the project, working to remove barriers and support engagement.
