'Game Plan', a new intervention project, focuses on empowering 18 to 25-year-olds who face challenges with their mental and emotional wellbeing.

Offering accessible community-based provision that complements Crisis Teams and Adult Mental Health Services, the programme has been designed to help meet increased demand for intervention services within communities, entailing activities that are bespoke and beneficiary-led. 'Game Plan' will support vulnerable adults to develop the skills, confidence and knowledge to thrive.

The role will operate an internal and external referral process that will see the Health and Wellbeing Officer working with key staff members and partners, including Lancashire and South Cumbria NHS Foundation Trust to support pathways and successful onboarding into the project, working to remove barriers and support engagement.
